Accu-Chek Aviva Plus Recall

Roche, the manufacturer of Accu-Chek® Aviva Plus test strips, has notified Humana Pharmacy, Inc. that one (1) lot of Aviva Plus test strips may not work as intended. The test strips from this lot may cause your blood glucose meter to give an error message and won’t allow you to test your blood sugar.

Again, this issue is isolated to one (1) lot of Accu-Chek® Aviva Plus test strips dispensed by Humana Pharmacy, Inc. The affected lot is 498315.

You can determine if your bottle of test strips is from this lot by checking your bottle of test strips. Please see picture below for location of lot on the bottle.

If your test strips are from lot 498315, Roche is advising to stop using the test strips from that lot and Roche is offering to directly replace your bottle of test strips at no cost to you.

  2. Email Accu-Chek Customer Care at accu-chek.care@roche.com and include the following information: Your name, full mailing address, phone number, test strip lot number, and number of affected test strip bottles
  3. Contact Accu-Chek Customer Care at 1-800-358-4866 Monday through Friday between hours of 8:00 a.m. and 8:00 p.m., Eastern Standard Time
