Accu-Chek Guide blood glucose meter recall from manufacturer

Roche, the manufacturer of Accu-Chek® Guide blood glucose meters, has notified Humana Pharmacy, Inc. that some Guide blood glucose meters may not work as intended. The affected Guide blood glucose meters may have power issues, battery issues, or may show an error message and won’t allow you to test your blood sugar.

This issue is isolated to Guide blood glucose meters that have a serial number less than 92311000001.

You can determine if your Guide blood glucose meter is impacted by looking at the serial number. You can find the serial number by checking the back of your meter. Please see picture below for where to find the serial number on your meter.

If your Accu-Chek Guide blood glucose meter has a serial number less than 92311000001 and you are experiencing power, battery, or error message issues, Roche is advising to do the following:

If meter is powered on and displaying an E-9 error or low battery icon:

  • Move the meter to a more moderate environment, such as ambient room temperature, to ensure optimal battery performance.
  • Turn the meter off, then turn the meter back on.
  • If the problem continues, perform a meter reset by doing the following:
    • Remove the batteries.
    • Press and hold the Power/Set/OK button for a least 2 seconds. 
    • Reinsert the batteries. 
  • If the problem still occurs after performing a meter reset, then the new ones according to the instructions in the user manual. Use high quality batteries in the meter, such as Panasonic. The meter requires two CR2032 batteries. 
  • If replacing the batteries does not correct the situation, please contact Accu-Chek Customer Care team through Accu-Chek’s website (www.accu-chek.com), email address (accu-chek.care@roche.com, or call 1-800-858-8072.
